LIABILITY

In anticipation of participating in wellness coaching, yoga classes or other yoga and shiatsu-related activities (the "Classes") offered by Revolution Motherhood, LLC, I hereby acknowledge that I am entering into Revolution Motherhood and fitness classes of my own free will. I acknowledge that Revolution Motherhood is not offering psychological therapy and does not prescribe or advise in any capacity the consumption of medication (prescription or over the counter). I acknowledge that I am creating all of my own goals and choose to follow through or not of my own free will and hold harmless Revolution Motherhood and its employees for any repercussions that may arise from my own actions during or outside of our meeting times. In anticipation of participating in Revolution Motherhood classes, yoga classes and shiatsu, I acknowledge that there are certain risks and dangers inherent in the physical practice of yoga and physical exercise, including a risk of mild or serious physical injury. I acknowledge that some of these risks cannot be eliminated regardless of the care taken to avoid injuries. In consideration of being allowed to participate in the Classes offered by Revolution Motherhood, LLC:

I hereby: (1) agree to assume full responsibility for any and all injuries or damages, including aggravation of pre-existing injuries, that I sustain in connection with my participation in the Classes ; and (2) release and hold harmless Revolution Motherhood, LLC from any and all responsibility, damages, liabilities, actions, costs and expenses arising out of or in any way related to my participation in the Classes to the fullest extent permitted by law, including for any liabilities or injuries resulting from the negligence of Revolution Motherhood, LLC and its members, principals and employees.

ALL SALES FINAL

 Your purchase is final, non-refundable and non-transferable. All make- up classes must be completed within your Revolution Motherhood Program schedule.

PRIVACY

All information shared by you may be used for teaching, marketing, and learning purposes. Your identity will be kept to your first name only.  We will never sell your personal information to a third party.
